MINOR HOCKEY PLAYER MOVEMENT FORM
The following is the detailed process for a minor hockey player/family to follow when requesting permission for Player
Movement. This Player Movement process can be accessed for any of the following scenarios:
• There is no Team in your age Division within your Resident Association
• Resident Association has a FULL team (17 skaters, 2 goaltenders) at your age Division
• Resident Association and another Association have joined together to form a team.
• Movement by Special Exception
o Movement to an MHA of equal or smaller Categorization
o Movement to an MHA of higher Categorization
o Moving to a Different Stream of Hockey
(Female Hockey, Non-Body Checking Hockey, Recreational Hockey)
PARENT DECLARATION FORM
The Parent Declaration Form is the designated Hockey Alberta form used by Players whose parent(s)/legal guardian(s) change residence in situations where the Player continues to reside with the parent(s)/legal guardian(s). The form is a declaration by the Parent(s)/ Legal Guardian(s) of the Player with regard to their permanent or primary residence.
The completed form is submitted to the Minor Hockey Association (c/o Registrar) for which the Player is registering.
FSMH UNDERAGE APPLICATION
Players that are of U9 age or higher who wish to play in a higher age category for the next hockey
season MUST complete an underage application form and submit the completed form to the FSMH
office by January 15th along with a non-refundable underage application fee of $150.00.
OVERAGE REQUEST FORM
NOTE: The deadline to submit an Overage Player Request Form is October 31st.
Hockey Alberta may allow a Player that is of first year in a division, by date of birth, to register and participate with a team or a lower age division provided they meet the criteria listed below. Approval of all Overage Players must be obtained through the application made by the resident Minor Hockey Association to the appropriate League President/ designate and Hockey Alberta Minor Leagues or Minor Female Committee.

U11 HADP- NOTIFICATION OF TRYOUT
The purpose of the U11 Hockey Alberta Development Pilot (HADP) – Notification of Tryout Form is to allow access to U11 HADP programming for those who do not have access to a U11 HADP team within their Resident MHA. Players who do not have access to a U11 HADP team(s) within their Resident MHA will be permitted to pursue a U11 HADP tryout with an U11 HADP hosting MHA that is accepting tryout players.
MALE ELITE PLAYER MOVEMENT FORM - a player who wishes to move from his permanent residence to play for an Elite team outside his resident draw zone.
THE APPLICATION PERIOD FOR THIS FORM IS MAY 1 TO JULY 1 EACH YEAR. APPLICATIONS OPEN MAY 1, AND MUST BE RECEIVED BY HOCKEY ALBERTA NO LATER THAN JULY 1. NO APPLICATIONS ARE ACCEPTED OUTSIDE OF THIS TIME PERIOD.
THESE ELITE PLAYER MOVEMENT PROCEDURES AND CORRESPONDING FORMS ARE FOR U18 AAA, U17 AAA, AND U15 AAA ONLY.

LETTER OF PERMISSION
This form is intended for tracking players attending and participating in sanctioned camps, tournaments, and events. This form does not give permission to try out.
When approved by your Minor Hockey Association or Club Team President/Designate, this form grants permission for the below named player to attend and participate in the event outlined below.
It is understood by all parties that the below named player will return to the Minor Hockey Association/Club Team issuing this Letter of Permission.
Once all permission signatures have been provided, the Minor Hockey Association/Club Team hosting the sanctioned event will require a copy of the approved form. Submit the approved form as directed by the host Minor Hockey Association/Club Team.
